Whether you are new to the Seattle metro area or a long-time resident looking for a new dental provider, you will be sure to find our office unique among dental practices. Below you will find all of the required new patient forms you will need prior to beginning treatment at our facility. You can download the prerequisite forms in advance by selecting the PDF files below. Please complete each new patient registration form,the appropriate health history form, and bring them with you to your first appointment. Please remember to have your current insurance card so we can make a copy to file with your records and bill for services rendered.

Insurance:
As a courtesy service, our friendly, helpful staff will be happy to check your insurance in advance and let you know what your insurance will cover in advance. To better serve everybody please note that it is the patient's respondsiblity to inform us of any changes in their job,dental insurance carriers, plan, benefits, or previous usuage. If you have any questions regarding your dental insurance call our office at (425) 837-0383. We accept most dental insurance programs used by major employers in our area.
Payment information:

• Estimated Co-payment is due at the time of service.This is payable by cash, check or debit/credit (VISA, MasterCard, Discover).
• In most cases, patients will be billed for any outstanding account balance after the insurance portion of billing is met. (This is because the copay is an estimate only)
• Financial payment plans available.
